# How to create a performance review template?

To create a performance review template, first you need to navigate to the settings page, this can be accessed at the very bottom of the navigation bar on the left hand side. Next scroll to the performance management section and click the 'create a performance review template' button.

Next, we fill in the general form information.

There are 3 fields to be filled in.

- **Enable performance template:** Enable whether or not the template appears as an option when scheduling a performance review.
- **Name:** Give the name of the template.
- **Description:** Give a description about who and what the template is for.

Next fill in the employee and manager questions. The employee will need to answer the employee questions and the manager will need to answer the manager questions.

There are 2 fields to be filled in.

- **Question:** Fill in the question that you would like to be answered.
- **Weight:** Weighting for the question. A lower weighting will cause the question to contribute less to the score whereas a higher weighting will cause the question to contribute more to the score.

Afterwards, click create template and your performance review template is ready to go!
